Prostatic abscess

Presentation

1 week of fever and dysuria.

Patient Data

Age: 75 years
Gender: Male

Enlarged prostate, measuring 51 x 50 x 55 mm - volume approximately 76 cc.

The gland is polylobulated with cystic formations and with mobile internal echoes.

Bladder is normal.

The prostate is enlarged and it is almost replaced by multiple collections of peripherally enhancing, low-attenuation fluid.

Distended bladder with right ureterohydronephrosis.

Bilateral cortical renal cysts.

Hyperdence gall stone.

Small right pleural effusion.

Consolidation along with right middle lobe.

Case Discussion

Prostatic abscess is a rare entity, more frequent in men aged between 50 to 60 years and with risk factors such as diabetes or immunosuppression.

On ultrasound it can be seen as a hypoechoic, irregular and septated lesion, which may have increased Doppler flow at its periphery.

On CT it appears as hypodense collections with peripheral enhancement.

This case required transrectal drainage and intravenous antibiotic, with an adequate final evolution.
