A calcaneal intramedullary lucent lesion with a narrow zone of transition and intralesional calcific density is suggestive of calcaneal lipoma.
Calcifications at the insertion of the Achilles tendon.

A well-defined lytic lesion with central calcification is the classic appearance of calcaneal intraosseous lipoma, known as the cockade sign.
