Colorectal anastomotic leak

Case contributed by Mohammad Taghi Niknejad
Diagnosis certain

Presentation

Known and operated case of rectosigmoid cancer.

Patient Data

Age: 55 years
Gender: Male
Fluoroscopy

Post-operative changes are seen due to rectosigmoid resection, colorectal anastomosis and right-sided ileostomy. By injecting water-soluble contrast medium through the remnant rectum a small volume of the contrast medium leaks from the anastomotic site and accumulates in the pre-sacral space.

No significant stricture in the anastomotic site and the injected contrast media easily passes proximal to the anastomosis and fills the visualized left hemicolon.

Case Discussion

The colonic anastomotic leak can occur in the early or late postoperative phase due to anastomosis failure and is associated with high morbidity and mortality.

The use of barium sulfate contrast medium is contraindicated because if there is a leak and spill of barium into the peritoneal cavity it may cause chronic chemical peritonitis.
