Deep pelvic endometriosis with endometriotic cyst

Presentation

The patient presented with left loin pain, and a computed tomography (CT) kidney-ureter-bladder (KUB) scan revealed a large left pelvic cyst. The patient was referred to gynecology, who ordered an MRI gynecology pelvis workup.

Age: 30 years

The uterus body shows diffuse low T2 signal and a globular appearance, consistent with diffuse adenomyosis.

A large left pelvic endometriotic cyst with clear T1 and T1 FS bright content.

Low T2 signal serosal deposits along the sigmoid colon, best seen on the coronal images, keeping with serosal deposits.

Chronic cicatrizing changes along the posterior surface of the uterus with ovaries pulled medially.

The left round ligament is thickened.

Case Discussion

This is a good case showing deep pelvic endometriosis with multiple serosal deposits along the colon and chronic pelvic cicatrising changes secondary to endometriosis. Also, a large left endometriotic cyst and diffuse adenomyosis.
