Distal biceps tendinosis with bicipitoradial bursitis

Diagnosis almost certain

Presentation

Left elbow anterior pain - few days. No trauma.

Patient Data

Age: 45 years
Gender: Male

Thickening and mild heterogeneous echopattern of the left distal biceps tendon (better appreciated in the short-axis view of the tendon). An anechoic fluid rim around the distal tendon. No elbow joint effusion. The rest of the elbow scan was normal.

The contralateral asymptomatic elbow was examined for comparison. It shows a normal distal biceps tendon without bursitis.

Case Discussion

The patient who was active in the gym, presented with anterior elbow pain. He was unable to do a biceps workout. Clinically distal biceps tendon was palpable. The ultrasound shows distal biceps tendinosis along with bicipitoradial bursitis.

Unlike the proximal long-head biceps tendon, the distal biceps tendon does not contain a tendon sheath. So fluid around the distal tendon should not be labeled as tenosynovitis.
