Dural venous sinus thrombosis

Diagnosis almost certain

Presentation

Severe headache.

Patient Data

Age: 30 years
Gender: Male
mri

Distended right transverse and sigmoid sinuses with intraluminal signal change appearing of low signal on T2 and high signal on T1 and FLAIR in keeping with right sided dural sinus thrombosis.

Both optic nerves are tortuous with mild optic sheath hydrops with indentation of the posterior sclera at optic discs and partial empty sella, suggestive of secondary intracranial hypertension.

High signal intensity of the right middle ear and mastoid, suggestive of otomastoiditis.

Case Discussion

Acute dural sinus thrombosis involving right transverse and sigmoid sinuses with signs of secondary intracranial hypertension. It may resulted as a complication of right otomastoiditis.
