Heart-shaped patella (Rorschach radiology)

Presentation

Crush injury, a gate fell onto the patient. He presents with right knee pain and swelling.

Patient Data

Age: 20 years
Gender: Male

An unusual heart-shaped patella, a likely normal variation in shape.

There is pre-patellar and superficial infra-patellar bursitis.

The CT scanogram suggests a bilateral heart-shaped patella.

There were no fractures, dislocations or subluxations of the right knee on CT imaging.

Case Discussion

The shape of the patella can be described using the Wiberg classification. This is based on medial and lateral patella facet asymmetry and hence best assessed on axial plain film (skyline projection), CT or MRI views. The shape of the patella (facets) may influence patella tilt and predispose to patellar instability1. in this instance the significance of bilateral heart shaped patellae is uncertain.
