Hitchhiker thumb deformity - rheumatoid arthritis

Presentation

Arthralgia of both hands with deformities.

Patient Data

Age: 40 years
Gender: Female

Both hands (AP view)

x-ray

Bilateral hitchhiker thumb deformity (Z-thumbs), seen as flexion of the metacarpophalangeal joint and hyperextension of the interphalangeal joint.

Periarticular osteopenia is noted.

Case Discussion

The patient has had complaints for 7 years and was diagnosed with rheumatoid arthritis one year ago and is now on treatment (methotrexate and hydroxychloroquine). There was ulnar deviation of metacarpophalangeal joints that was corrected on left hand.

Hitchhiker thumb is one of the musculoskeletal manifestations of rheumatoid arthritis, although is a non-specific finding.
