Ileocolic intussusception - lipoma lead-point

Presentation

Cecal mass on colonoscopy.

Patient Data

Age: 70 years
Gender: Female

Ileocolic intussusception with the lead point a well-circumscribed round fat-density mass.

No dilated loops of bowel. Sigmoid diverticulosis.

Metalware from a lumbar fusion.

Ileocolic intussusception with the lead point a high T1/T2 signal well-circumscribed round mass with homogeneous fat suppression in keeping with a lipoma. Minimal internal vasculity. No internal septation or nodularity. No restricted diffusion. Mild mural thickening and edema of the intussusceptum but homogeneous mural enhancement.

Sigmoid diverticulosis. No other bowel mass is identified. No dilated loops of bowel. No intraperitoneal free fluid. No enlarged lymph nodes.

Previous 1 L4-5 and L5-S1 lumbar fusion.

Case Discussion

This is clearly a pathological intussusception, given the large mass that has features of a lipoma. The patient was treated at a different hospital and confirmatory histopathology is not available.
