Lithium-induced nephropathy

Presentation

Hematuria. History of bipolar disorder.

Patient Data

Age: 60 years
Gender: Female

Kidneys: Normal in size.  Both display numerous tiny echogenic cortical foci displaying twinkle artifact. Otherwise normal, without any large focal lesion or hydronephrosis.  

Discussed with the referring clinician, who confirms the patient is on long term lithium therapy.

The kidneys are diffusely abnormal, both displaying very numerous sub 5 mm hypodense cortical  lesions without enhancement, consistent with microcysts.  No renal calcifications. No abnormality of the renal parenchyma or collecting systems.  Only the proximal
