Nasogastric tube in the right lung. Bibasilar opacity may represent atelectasis. No pneumothorax.

Case Discussion

Appropriately positioned nasogastric tube tips are located inferior to the diaphragm. In this case, the NG tube is in the right lung. Inadvertent pulmonary placement more commonly involves the right lung compared to the left lung because the right bronchus is the more direct continuation of the trachea. A common complication of pulmonary NG tube placement is pneumothorax and a repeat radiograph after removal/repositioning of the NG tube should be obtained to confirm appropriate tube placement and to exclude pneumothorax.
