Prolapsed pedunculated uterine leiomyoma

Presentation

Menorrhagia with a cervical mass on gynecological examination.

Patient Data

Age: 45 years
Gender: Female

Deformed uterus with a cervical prolapsed pedunculated mass arising from the uterine fundus. It measures 50 x 45 x 42 mm (excluding the pedicle). This mass has an intermediate to low signal on T2, without restricted diffusion.

The inferior part of the mass demonstrates heterogeneous enhancement with an area of restricted diffusion (still less than that of the endometrium).

The transitional zone is thickened measuring up to 12 mm, but there are no micro cysts or hemorrhagic spots.

There is no endometrial thickening.

There are many other round-shaped myometrial masses, demonstrating a low signal on T2 images, in keeping with hyalin leiomyomas.

The ovaries are unremarkable.

The patient had a biopsy which revealed surface-ulcerated hyalin leiomyoma.
