Sigmoid carcinoma with diverticulitis

Case contributed by Essam G Ghonaim
Diagnosis almost certain

Presentation

Patient presented with rectal bleeding and lower abdominal pain.

Patient Data

Age: 45 years
Gender: Male

There is asymmetric colonic wall thickening seen involving a short segment of sigmoid colon, suggestive of a neoplastic process. There is an associated bubbly appearance of multiple rounded air-containing diverticula (in keeping with colonic diverticulosis) as well as peri-colic fat stranding and poorly-defined area of hazy increase in attenuation value of mesenteric fat (in keeping with diverticulitis).

Case Discussion

Endoscopic biopsy revealed a colonic adenocarcinoma and diverticulosis. Colectomy was carried out. Histopathology revealed adenocarcinoma and diverticulitis.

While there is no association between colorectal cancer and colonic diverticulosis 1-3, there is evidence of possible increased risk of colorectal cancer in patients with diverticulitis 4,5.
