Subacute thyroiditis

Diagnosis almost certain

Presentation

Patient presented with 2 week history of tremors and palpitations. The patient's GP had performed bloods which were positive for thyroid peroxidase auto-antibiodies (TPO)

Age: 50 years
Gender: Unknown

Tc-99m pertechnetate scan

Nuclear medicine

No activity is seen in the thyroid gland on the planar anterior images.

Although the neck to thigh ratio is mildly elevated using institutional ranges (0.5-2.0), this is not an accurate assessment given the thyroid gland is unable to be visualized accurately.

These scan findings are consistent with subacute thyroiditis.

Case Discussion

Even though the scan findings point towards a diagnosis of subacute thyroiditis, these findings can also be seen in patients who have received IV Iodinated Contrast within the previous 8 weeks.

The patient had a fairly recent clinical history of multiple viral illnesses which was deemed the cause of the subacute thyroiditis.
