Enlargement of extraocular muscles sparing tendinous attachments, specifically rectus muscles. Hypertrophy of extraconal fat and bilateral proptosis are noted.

Case Discussion

The patient has a history of uncontrolled thyrotoxicosis, with these typical CT features of thyroid ophthalmopathy.
