A well-defined rounded midline mass lesion, originating from the tuber cinereum, exhibiting a homogenous isointense signal to the grey matter in T1 and T2, and shows no significant enhancement in the post-contrast study. It indents the floor of third ventricle, basilar and posterior cerebral arteries, optic chiasm and pituitary stalk.

Case Discussion

Hypothalamic hamartomas are benign pediatric homogeneous non-enhancing brain masses that occur in hypothalamic region arising from the tuber cinereum. They usually present with gelastic seizures, visual problems, precocious puberty and cognitive disability. Differential diagnosis is the non-enhancing hypothalamic glioma.
