Unilateral sacroiliitis

Diagnosis almost certain

Presentation

Left hip pain for four days. No fever or trauma.

Patient Data

Age: 25 years
Gender: Male
mri

Bone marrow edema (hyperintense in STIR and hypointense in T1) in the lower half of the left sacroiliac joint with mild erosion in the adjacent bones.

Mild soft tissue swelling adjacent to the sacroiliac joint.

No subchondral sclerosis and cystic changes.

No abnormal signal within the muscles or abscess formation.

The right sacroiliac joint appears normal.

Case Discussion

In a case of sacroiliitis with unilateral sacroiliac joint involvement and adjacent soft tissue swelling, this raises the suspicion of infective sacroiliitis. Other possible causes include spondyloarthropathy-associated or mechanical stress.
