There is a hypoechoic area noted in the uterine isthmus with discontinuation of the myometrium at the site of the previous Cesarean scar. It measures 1.4 x 1.0 x 0.5 cm.
Both ovaries are normal (not shown).
Case Discussion
The anechoic fluid collection representing blood in a Cesarean scar niche can be the cause of the dysfunctional uterine bleeding, which explains the vaginal spotting observed in this instance following a regular menstrual cycle.
