Presentation

Headache and heaviness of head

Patient Data

Age: 40 years
Gender: Female

MRI with contrast

A well-defined lobulated mass (about 32 x 29 mm) which is moderately hyperintense in T1WI and T2WI and intensely hyperintense in FLAIR images is seen within the sphenoid sinus. The lesion shows facilitated diffusion on DW images. On post-contrast images, no significant enhancement is seen. The mass is expanding the sphenoid sinus and abutting the left optic nerve. The pituitary gland appears normal.

Case Discussion

High T1WI signal suggests protein rich materials. Expansion of the sphenoid sinus and protein rich content suggest the diagnosis of a protein rich sphenoid sinus mucocele.

Paranasal sinus mucocele is a condition when the sinus is filled and expanded by mucous due to chronic obstruction.
