Ileocaecal lymphoma

Presentation

Acute right lower quadrant pain. No surgical history.

Patient Data

Age: 20 years
Gender: Male
ct

Large luminal mass in the caecum, appendix and terminal ileum. The mass is relatively homogeneous with few scattered central calcifications. Mildly dilated small bowel proximal to the mass. Adjacent large, conglomerate mesenteric lymph nodes.

Case Discussion

Biopsy results showed diffuse large B-cell lymphoma. Other considerations include adenocarcinoma and carcinoid. Large tumour size and modest obstruction, along with patient demographic, favour lymphoma over adenocarcinoma. Absent desmoplastic reaction and conglomerate mesenteric lymph nodes favour lymphoma over carcinoid.
